2025 Player/Waiver form for all divisions

This is just a reminder that all returning and new players must fill out the Player Wavier form on the CDMSBL web site. This must be done before the start of the 2025 season. If you play in more divisions, you must fill one out for every division that you plan to play in. After you complete the form, an email will be sent to you confirming that you have completed it. Harold, FYI to all league members, the CDMSBL Forms link displays the Waiver Form as "2024 Waiver Form", however when you click on the of the it, you are directed to the 2025 Waiver Form.
